Expenses Claim Form

The way we can pay your expenses has recently changed. You may still receive a cheque to your home address if you wish, however we are now able to put monies into your account by bank transfer.

If you wish to be paid straight into your bank account, please provide your bank details on the expenses form. Otherwise leave this section blank. If you have a stash of old expenses forms you need to use up, these will be accepted and your bank account details should just be added somewhere on the page.
